NDIS Employment, Education and Life-Stage Transition Support in Perth

The big changes are where people lose ground: leaving school, starting work, moving out, leaving hospital or custody. We make them a plan with steps you can see.

Practical, time-limited support through a change

Prevail Care is registered to support life’s transitions under three Capacity Building groups: Assistance to Access and Maintain Employment or Education, Assistance with Life-Stage Transitions, and Accommodation and Tenancy Assistance.

The work is practical. One sentence defines the goal, we map the steps and the other people involved, a regular worker does the steps with you week by week, and support reduces when the goal is reached.

Employment and education support

Not a job agency. Disability Employment Services is a separate Commonwealth program that finds jobs and works with employers. NDIS employment support is for the capacity you need to get there and stay there:

  • Work readiness, routines and getting up on time
  • Travel training to the workplace or campus
  • On-the-job support while you learn the role, where the employer agrees
  • The paperwork of starting work: tax file number, bank, superannuation, Centrelink reporting
  • Managing the social side of a workplace or a classroom
  • Enrolling at TAFE or university, settling in and keeping up with the structure of a course

We work alongside your DES provider, employer or school, never instead of them.

Life-stage transitions we support

Leaving school

Building a weekly routine that replaces the school day, trying work experience, learning transport.

Moving out

Choosing a housing option (SIL, ILO, private rental, public housing), applying, setting up the house, learning to run it.

Leaving hospital

Discharge planning with the ward, re-establishing supports at home, medication routines.

Leaving custody

Pre-release planning, a place to live, ID and Centrelink reinstated, supports in place on day one.

A change in family care

When a parent or carer can no longer continue, planning the next arrangement without a crisis.

Keeping a tenancy

Applying for housing, understanding the lease, dealing with the landlord, meeting the obligations so the tenancy does not fail.

How it works

  • Define the transition. One sentence: “living in my own place by June”, “working two days a week”. Everything is planned back from it.
  • Map the steps and the people. Who else is involved (DES, school, hospital, Corrections, housing), what they do, what we do.
  • Weekly sessions. A regular worker, usually two to four hours a week, doing the steps with you, not for you.
  • Hand over. When the goal is reached, support reduces to a check-in or stops. Progress is reported to your coordinator for the plan review.

Questions about transition support

No. DES finds jobs and is funded by the Department of Social Services. NDIS employment support builds your capacity to get and keep the job. You can have both at the same time.

Yes, under Accommodation and Tenancy Assistance, including the application, the inspections and keeping the tenancy once you have it.

Twelve months out is ideal, so the plan review can include the funding. We can meet the school and the family together.

Yes. It is a specialty. See our justice support services page for how pre-release planning works.

No. It funds the support to get you there and keep you going. Fees are yours or covered by other programs.

Capacity Building: Finding and Keeping a Job, Improved Living Arrangements, or Improved Daily Living, depending on the goal.

Two hours, usually once or twice a week for a set period, then reducing as the goal is reached.
