Support to take part, not a program to fit into

Community participation funding pays for a worker to help you get out and take part in ordinary life. Prevail Care delivers it one-to-one across Perth, matched to what you actually want to do rather than to a centre timetable.

It is funded from Core Supports under Assistance with Social and Community Participation, and we are registered under Participation in Community, Social and Civic Activities, Innovative Community Participation and Group/Centre Activities. Where two or three people want to do the same thing, we can roster one worker for a small group at a lower rate per person.

What a community support worker helps with

  • Sport, the gym, recreation, clubs and classes
  • Appointments, including medical, legal and Centrelink, and understanding what was said
  • Shopping, banking and errands, with support to do them yourself
  • Visiting family and friends, events, concerts, the beach, the footy
  • Getting to and settling into work, TAFE or university
  • Learning public transport, with the worker fading back as you get confident
  • Volunteering and community groups
  • Big days: skydiving over Rottnest, fishing at dusk, a round at the driving range. Our participants have done all three.

★★★★★

“The most amazing and supportive team. We get continuous contact and updates on support, with the team being extremely proactive in searching for and trying new ideas and supports. They have said and have proven to be there for us night and day. They have changed our lives.

“They are respectful, professional and overall my top pick for NDIS services for support workers. โ€ฆ They work hard to find the right fit for my needs as a person living with multiple disabilities. โ€ฆ The efforts I have seen have made it a really clear decision to stick with them as a service.

Who community participation is for

Anyone whose plan includes social and community participation funding: young people leaving school, people with psychosocial disability rebuilding confidence after a long time at home or in hospital, people with intellectual disability or autism who want a regular weekly routine of activities, and people in SIL who want time out of the house with someone who is not a housemate.

A lot of our community work is with participants whose anxiety, behaviour or history makes crowded places hard. Workers plan for that, know the quiet entrances and the exits, and leave early without drama when that is the right call.

How we plan the support

Goals from the plan

We start with the goals the planner wrote and the ones you actually care about, which are not always the same.

A weekly pattern

Most people do best with the same activity on the same day with the same worker. We build that and leave room for one-offs.

Fade where you want to

If the goal is to do something alone eventually, each outing is a step toward it and we record what you did without help.

Report back

Shift notes record what happened and what to try next. Your coordinator gets a summary before each plan review, with your consent.

Community participation questions

Yes, where you agree to it and the plan funds transport. Workers’ cars carry full comprehensive insurance and the kilometre rate is the NDIS rate, written in your service agreement.

You pay your own entry fees, meals and tickets. The NDIS pays for the worker’s time. Your worker’s entry is covered by your Companion Card wherever it is accepted.

Yes. Evening, Saturday, Sunday and public holiday rates apply, as they do with every provider.

Yes, at a 1:2 or 1:3 rate, which costs each person less. Everyone sharing must agree.

Support worker time on a holiday can be funded; the travel, accommodation and the worker’s costs usually are not. Talk to your coordinator first and we will help with the request.

Seven days, waived in extenuating circumstances. If we cancel, you are never charged.

Whatever your plan funds. We help you work out how far the budget goes at weekday, evening and weekend rates.
